We had the good fortune of connecting with Yasmilka Sarmiento and we’ve shared our conversation below.

Hi Yasmilka, what was your thought process behind starting your own business?
Gardenia Medical Care has evolved from an impactful journey in healthcare, born out of my professional experiences and the desire to serve underserved communities. With my background as a Physician Associate, I’ve witnessed firsthand the significant challenges that many individuals face in accessing quality care, particularly those in rural, Spanish-speaking, and financially constrained communities. Our work began with community-driven initiatives, like workshops and health screening fairs, which allowed a strong understanding of the needs that go beyond just medical treatments. We have been able to identify not only a gap in healthcare access but also a lack of education surrounding important medical conditions, which can make it even harder for patients to manage their health. Our long-term commitment to addressing the barriers these patients face—whether it’s language, education, or financial limitations—speaks for itself in Gardenia’s dedication to both the medical field and the well-being of our patients.

I’ve thought deeply about both the systemic issues, and the solutions needed to make healthcare more accessible, affordable, and equitable. Creating a cash-only concierge practice in underserved communities is an innovative approach that addresses several key challenges simultaneously: the lack of insurance, the scarcity of bilingual providers, and the need for more personalized care. By focusing on affordability and offering services like telehealth and medical coaching, we are making high-quality, compassionate healthcare more accessible to populations who are typically left out, especially in remote areas where access to providers is limited. Breaking down the barriers that often keep lower-income, Spanish-speaking communities from seeking care—namely, cost, language, and lack of knowledge about their own health. By reducing costs while still providing the personalized attention patients deserve, we’re challenging the traditional healthcare model, which often prioritizes volume. This also positions my practice as a model for how to bring effective, patient-centered care to areas that are often overlooked by the mainstream healthcare system.

We have been able to adapt Gardenia Medical Care to meet a broader range of needs, recognizing that healthcare challenges aren’t confined to rural or underserved communities alone. The fact is, that there are busy professionals and self-employed individuals who often face barriers to care despite having insurance. For people in suburban and urban areas with busy careers, the flexibility of telehealth and concierge services is a game-changer. Many patients today find it difficult to balance work, family, and personal health, so offering a model where they can receive high-quality, personalized care without disrupting their schedules is a significant benefit. Similarly, self-employed individuals or those with high deductibles/copays often feel financially burdened by traditional healthcare costs. Offering tailored medical plans that meet their specific needs could make a huge difference in both their access to care and their ability to manage their health affordably. Developing different medical plans allows us to meet people where they are—whether they need the flexibility of telehealth, the affordability of concierge care, or a more accessible option for those without insurance.

Our goal to move away from healthcare disparities and towards healthcare equity is not just about offering services but also about empowering patients. The approach we’ve taken, combining medical care with education, is crucial for long-term health improvement. This isn’t just about treating symptoms—it’s about teaching patients to understand their health, make informed decisions, and ultimately become their own health advocates.

Alright, so for those in our community who might not be familiar with your business, can you tell us more?
At Gardenia Medical Care we believe that healthcare should not be a privilege, it is a right. Everyone should have access to it regardless of their background, financial situation, or location. Too often I’ve met patients who delayed or avoided medical care because it was too expensive, too far, or simply too complicated to access. That realization became the driving force behind why I built this practice- to remove the barriers that keep people from getting the care they deserve. Whether that’s language or financial constraint. As a Physician Associate (PA), I don’t just see patients—I see people with stories and real struggles. I see the single mother who works two jobs and can’t afford to take time off for a doctor’s appointment. I see an elderly patient who has difficulty traveling but still needs to be cared for. I see the hardworking individuals who put their health last, because they cannot afford to take care of themselves and feel like they have no other choice. I built Gardenia Medical Care for them.

What sets us apart is having a hybrid model of concierge medical coaching and telehealth services to bring healthcare directly to those who need it most, those who are often overlooked. Our mission is simple: to make healthcare affordable and accessible. One of the things I’m most proud of is how we are helping patients feel heard and understood. Whether it’s by providing medical coaching, specialized services or going the extra mile to make sure our patients have the proper care, we’re changing lives by reinventing concierge medicine. Every day, I get to witness the impact of compassionate, personalized care. Hearing the gratitude in the patients’ voice when they no longer feel dismissed or unheard—that is what keeps me going.

The journey of building Gardenia Medical Care hasn’t been easy. It required courage, sacrifice, perseverance, and an unwavering commitment to serving people. I come from a background that values hard work, resilience, and service to others. Growing up, I witnessed how access to quality healthcare—or the lack of it—can deeply impact lives. This coupled with the experiences lived while being a “disadvantaged” college student also developed my passion for mentoring and educating others. All of this shaped my commitment to bridging healthcare gaps and creating a medical practice where patients are heard, understood, and cared for with compassion. Furthermore, where they are not just treated for their condition but educated, guided and empowered to be the healthiest version of themselves.

The biggest challenge I’ve had to face was fear of the unknown. However, when you have set goals and purpose those things drive you to move forward and take that risk. As Kylie Francis said “You will never reach your true potential living life in comfort and routine. You’ve got to take some risks”. Starting Gardenia Medical Care was a leap of faith, but I knew that if I didn’t take the chance, I wouldn’t be able to serve patients in the way I truly envisioned. Was I scared? Absolutely! Were there times that I doubted if I could? You bet! However, my faith in God and that he will allow it to work out kept me calm. Once I decided to take that risk, the universe has aligned itself and allowed me to meet different people who have become my mentors and have pointed me in the right direction. Every challenge has been worth it because we are redefining what healthcare looks like—one patient at a time.

Throughout my career, I’ve learned that calculated risks—backed by preparation, knowledge, and a strong mission—often lead to the greatest rewards. Expanding into concierge medicine, offering specialized services and embracing telehealth are all risks that will pay off by allowing me to reach and help more people. I want the world to know that Gardenia Medical Care isn’t just a business—it’s a movement. A movement that says, “Your health matters. Your voice matters. If you had a friend visiting you, what are some of the local spots you’d want to take them around to?
If my best friend were visiting Georgia, I’d want to show them the best of what the state has to offer—a mix of culture, history, food, and nature. Here’s how I’d plan an itinerary filled with memorable experiences: Exploring Atlanta’s Landmarks- We’d start with the Georgia Aquarium, one of the largest in the United States, followed by the World of Coca-Cola to taste sodas from around the world. Later, we’d stroll through the Atlanta Botanical Garden for a peaceful escape surrounded by nature. A Journey Through History & Culture- We’d visit the National Center for Civil and Human Rights which I’m sure would be both inspiring and eye-opening. Afterward, we’d walk along the Atlanta BeltLine, enjoying its street art, local vendors, and vibrant energy. Outdoor Adventures & Scenic Views for some fresh air and adventure, we’d go hiking at Stone Mountain Park or visit Amicalola Falls State Park where we can find the tallest cascading waterfall in the southeast. Food is a huge part of any trip! We’d stop by Ponce City Market for a variety of great restaurants. For a stunning view of Atlanta’s Skylines, from Buckhead to downtown, 9 Mile Station is a must. Where we can taste delicious, American grill style cuisine. And if we’re up for nightlife, catching a live show at The Fox Theatre would be the perfect way to end the night. Shopping & Relaxation- We can browse The Shops of Buckhead or explore Atlantic Station for a mix of shopping and entertainment. To wrap up the trip, we’d unwind at Woodhouse Spa in Buckhead for relaxation and wellness treatments. Georgia is such a beautiful and diverse place with a lot to offer.

Who else deserves some credit and recognition?
I have been extremely blessed to have so many great people supporting me throughout this journey. I’ve been empowered by amazing women in my life. I would like to dedicate my shoutout to my friend, Jissel Fernandez, owner of Questus Group. A company who empowers healthcare innovators by developing customized strategies to help their business succeed. With her master’s in business administration from Duke University, Jissel is great at operating business plans. She believed in my vision and pushed me to take that leap of faith when I was too scared to see the impact that Gardenia Medical Care will have. Her expertise in business strategy has been incredibly empowering. Her support has profoundly impacted both my personal and professional growth. The other woman I’d like to shout out is my sister, Julissa Sarmiento, Executive Assistant of Gardenia Medical Care. Her support is invaluable- having someone I can rely on to manage the details allows me to focus on what matters most: providing excellent care for my patients. Julissa’s creativity helps me see beyond my comfort zone. Last, but not least I would like to dedicate this to one of my best friends, Yunuen Estrada Garibay MD, who was present in our first health fair in Mississippi laying the groundwork for Gardenia Medical Care. Her commitment to excellent patient care had a profound impact on shaping the values and mission behind Gardenia. Her dedication and passion for service continues to guide my team as we serve our communities. Naming the organization after her favorite flower is our way of keeping her powerful legacy alive.
